Insurance-

We accept most but NOT all insurance (always call to verify)

  • Most regular commercial insurances
  • No Medicaid, No Superior, No Wellpoint, No Molina
  • Limited Medicare (Prefer Traditional Medicare + Supplement)
  • Limited ACA/Marketplace insurance

Self Pay Option- (payment of $125 prior to being seen and other charges if incurred will be due at check out)

  • One problem – $125
  • Two problems – $150
  • Three problems – $175
  • Physical – $150-$200

(Fee includes physical exam only. Any ordered labs, imaging, vaccinations, or medications given are a separate/additional charge.)

Before requesting a refill, ask the pharmacy if you have a NEW RX waiting to be filled that was sent at your last appointment, you may have NO refills on your current RX BUT a NEW RX is waiting to be filled. Pharmacies tend to overlook this so please ask before requesting refills.

Patients are expected to use HEALOW APP

Functions- view results of labs/ imaging, referrals, pay bill, NON urgent portal messages, televisit, self-schedule/cancel appointments, check in for appointment, etc

Please cancel  appointments at least 24 hours in advance
Cancellations < 24 hours are considered late cancellations

  • 1st late cancellation or missed appointment (warning)
  • 2nd late cancellation or missed appointment $50 fee before scheduling next appointment 
  • 3rd late cancellation or missed appointment (patient may be dismissed from the practice)

We understand that emergencies do happen and will take that into consideration. We appreciate your understanding and cooperation. Keeping open schedules allows us to better serve patients who want to be seen.

Late Arrivals 
Established patients arriving  > 15 minutes late may need to reschedule their appointment

New patients arriving  > 15 minutes late  will be rescheduled

Medical advice through the portal may be billed to your insurance and result in a patient charge)

  • PLEASE allow 1-2 business days for a reply (excludes weekends and holidays)
  • PLEASE limit to 1 question per portal message
  • Portal messages are good for short refill requests (30 days- exclude controlled meds), administrative questions, one quick, simple question
  • Portal messages are NOT for new problems, medication changes, multiple questions. You will need to schedule an in-office appointment or televisit for this.

While televisit is convenient, NOT ALL medical issues can be addressed virtually. The provider decides when an in-office visit is needed.

Telehealth visits are available for established patients who have been seen in the clinic within the past 12 months. This is to ensure safe, accurate, and effective care. Patients overdue for an annual physical/checkup will be scheduled for an in-office visit

Reviewed in 7 days and called to patient by staff

Results are also in the patient portal/ HEALOW APP

If you do not receive your imaging or lab results in 7-10 days, please notify our office by calling or sending a portal message.

Please allow 7 days for processing.

Each insurance has different PA forms. Updated insurance is required to prevent rejection.

Referrals are not processed same day or next day. Please allow up to 7 days for referral processing.

We use insurance directories for referrals BUT they are NOT always up to date, ALWAYS call the specialist’s office to confirm they take your insurance

Some insurance companies have small referral networks. You may be asked to find your own referral.
